Repositioning of a dislocated IOL using the Lasso technique

  1. This patient has a completely dislocated IOL lying on the retina. The retina shows myelinated nerve fibers and old laser marks. Iris hooks have been used to get adequate pupillary dilation during surgery. A complete 20G vitrectomy is done. Chandelier light is used to facilitate bimanual maoeuvers. Partial thickness scleral flaps are made in the horizontal meridians. A double armed 10-0 prolene suture on straight needles is used. The video shows how to make the lasso loops. The second knot of the lasso is to fix the lasso loop itself. The size of the loops is adjusted and excess suture is cut. The loops are passed over the haptics and are tightened at the maximum curvature. The sutures lying outside the globe are pulled to lift the IOL and position it properly in the center of the pupil. The straight needle of the prolene is bent, passed through the bed of the scleral flap and the suture is tied onto itself. Flaps, sclerotomies and conjunctiva are sutured.
